BUILT FOR COMPLEX CASES

A Reliable, Responsive Service for Complex Cases

We understand the pressures of managing complex neurological cases where delays, poor communication, or inconsistent support can impact outcomes and the progress of the case. Our approach is designed to facilitate both rehabilitation outcomes and the wider demands of the case.

Clinical Expertise & Continuity

We match each client with an appropriate specialist neurological physiotherapist and aim to maintain consistency throughout rehabilitation, reducing unnecessary handovers and supporting a strong therapeutic relationship.

We respond promptly to referral enquiries and provide clear information from the outset. During rehabilitation, we communicate proactively with case managers and the wider MDT, rather than waiting to be chased.

Assessments and progress reviews are comprehensive and supported by clear clinical reasoning, objective measures where appropriate, defined goals and structured reporting to support informed decision-making.

Treatment can take place in the home, clinic, gym, workplace or community, depending on where rehabilitation is likely to add the most value. The focus remains on meaningful function and transfer into everyday life.

WHAT TO EXPECT WHEN YOU REFER

A Structured and Transparent Referral Pathway

1. Referral & Clinician Matching

We accept instructions for initial assessments and ongoing neurological physiotherapy during active litigation and post-settlement. Before allocation, we consider the client’s clinical presentation, goals and individual circumstances to identify an appropriate physiotherapist. Where useful, we can arrange a complimentary video meet-and-greet to support early rapport and clinical fit. Before instruction, we provide clinician CVs, fees, terms and relevant professional compliance documentation.

Following instruction, we complete a detailed assessment to establish the client’s current presentation, rehabilitation priorities and recommended physiotherapy provision. Reports are comprehensive and include injury impact, client aims, objective clinical measures, treatment and equipment recommendations, with projected costs. The aim is to give the case manager a clear clinical picture and a transparent basis for decision-making from the outset.

Rehabilitation is individualised, functionally focused and adapted as the client’s needs change. Depending on the case, treatment may involve home or community rehabilitation, clinic-based therapy, specialist technology, support-worker or family training, risk assessment and progression into work, exercise or other meaningful environments. Treatment frequency and recommendations are reviewed as rehabilitation progresses, with changes communicated clearly alongside the clinical rationale and likely cost implications.

We understand that good clinical work is only useful to the wider case when it is communicated clearly. You can expect structured progress reporting, proactive updates, timely responses to information requests and attendance at MDT meetings where required. Our reporting is designed to show what has been addressed, what has changed, what remains relevant and what we recommend next.

COMPLEX REHABILITATION IN PRACTICE

Different Presentations, Clear Rehabilitation Priorities

Our focus is always on achieving meaningful, measurable outcomes that support independence, reduce long-term care needs, and improve quality of life. We work closely with case managers and multidisciplinary teams to ensure rehabilitation is targeted, effective, and aligned with the wider objectives of each case. We support people with a wide range of complex neurological conditions, including following an acquired brain injury (ABI), spinal cord injury, a functional neurological disorder, or those who are experiencing vestibular dysfunction.

Traumatic Brain Injury (TBI) - Complex Community Rehabilitation

Presentation

The client suffered a moderate traumatic brain injury resulting in significant mobility, cognitive, and speech difficulties. They relied on carers for mobility within the home and were unable to safely access the community.

Working across the home, gym, and community, specialist neurological physiotherapy formed part of a coordinated multidisciplinary rehabilitation programme focused on restoring confidence, independence, and participation.

Progressed to independent mobility at home and supervision only in the community, alongside a return to the gym, exercise classes, Pilates and running in the park. Most importantly, rehabilitation helped restore everyday family life, including being able to get down on the floor and play with their child again.

“Emma, and her colleagues have been pivotal in the recovery that my client has made. They have taken her from always being dependent on the close support of carers, to being someone who is independently mobile…My client is now able to play on the floor with her child and attend the gym and exercise classes which she loves.”

Traumatic Brain Injury (TBI) – Meaningful Rehabilitation

Presentation

Following a severe traumatic brain injury, the client presented with significant cognitive and complex behavioural difficulties alongside mobility challenges related to visual, vestibular and lower limb musculoskeletal injury. Rehabilitation required a highly individualised and adaptable approach, delivered within the client’s home and community and coordinated closely with the wider multidisciplinary team.

Physiotherapy focused on using meaningful, activity-based rehabilitation to improve physical function while supporting confidence, engagement and quality of life. Therapy extended beyond traditional exercises, incorporating activities chosen around the client’s interests and goals, including cookery courses, adapted archery and basketball. Close collaborative working across the MDT ensured that rehabilitation remained consistent, responsive and centred around the client.

The client made significant progress in functional ability, independence and community participation, progressing from requiring substantial support and high-dose medication to manage behavioural and anxiety-related difficulties to purchasing his own property to move into. Activity became an important part of their rehabilitation journey, creating opportunities to develop physical ability, independence and participation while maintaining the client’s own goals and interests at the centre of the programme.

“Alice is amazing at collaborative working with the whole MDT and throughout her practice our client remains at the centre of it. She is imaginative in her practice and always ensures that the support she provides is completely personalised to our client and remains focused on achieving his chosen goals. I feel extremely lucky to be able to call Alice a colleague.”

Complete Spinal Cord Injury (Thoracic) - Community Reintegration

Presentation

Following a complete C4 spinal cord injury due to complications during surgery, resulting in complete loss of movement and sensation below the level of injury, the client was reliant on a wheelchair and specialist equipment for mobility and transfers.

Weekly home-based rehabilitation was provided, with close interdisciplinary working. The focus extended beyond physical rehabilitation and long-term disability management, to create opportunities for participation and independence.

Pre-existing pressure sores resolved through 24-hour postural management, including specialist sleep and positioning equipment, and close multidisciplinary working. Rehabilitation also supported a safe return to the community, visits to meaningful places and the development of a mouth-painting setup for independent creative expression. This restored choice, purpose and participation in everyday life.

“Being able to get out to places that matter to me and paint again has given me more choice in how I spend my days.”

Functional Neurological Disorder (FND) - Restoring Confidence, Mobility and Function

Presentation

Following a traumatic C2 spinal fracture, our client experienced persistent Functional Neurological Disorder symptoms alongside complex cervico-vestibular issues, PTSD-related symptoms, reduced community confidence, and significant deconditioning.

A structured rehabilitation programme provided as part of coordinated interdisciplinary intervention, including home-based therapy, gym rehabilitation, swimming, and community-based neurological physiotherapy.

Improved walking tolerance, confidence and physical capacity supported a return to independent gym attendance, swimming club participation and wider community activities, helping to rebuild independence, fitness and confidence in everyday life.

“I just wanted to thank you for all your help, guidance and support. From your explanation and understanding of this condition, my mindset has completely transformed and helped me to accept and deal with it all so much better… you’ve honestly turned my life around.”

CLINICAL GOVERNANCE & CONTINUITY

A Fully Employed Team of Specialists

Our clinicians are employed members of Physio4You within our NeuroPhysio service, rather than a fragmented network of independent contractors. That structure supports greater continuity of therapist, shared clinical standards, internal support, peer discussion and clearer accountability across complex cases. We also maintain professional compliance requirements including DBS checks, mandatory training, clinical supervision and appropriate professional insurance, with relevant documentation available. For case managers, the practical benefit is simple: a more consistent service, clearer communication and greater confidence that consistent, high-quality rehabilitation is being delivered and overseen.
